Excellent hospice care is about developing a relationship between caring, skilled practitioners and you. It is also family centered and believes in providing support to everyone who is affected by a debilitating diagnosis. The care team at Hospice de la Luz is passionate about caring.

Hospice de la Luz team members have years of hospice experience. Our nurses, physicians, certified nursing assistants, chaplains, social workers and volunteers share a common commitment for compassionate care and family support. We go beyond traditional medical care. In addition to treating the patient’s physical needs, we provide emotional and spiritual support for the entire family. We believe in supporting the whole person – body, mind and spirit. Hospice de la Luz is there when you need a helping hand. Our hospice care team is composed of:

NURSES: Often our nurses (case managers) will see our patients one to two times a week. They will check vital signs, medications, skin conditions, evaluate pain or treatments and spend time with the family and caregivers involved in the care. Our nurses provide information, education and TLC with each visit. Our patient’s primary nurse is available during regular business hours, but we also have registered nurses on call for emergencies, weekends and after hours. They are available over the phone and will also make a home visit as needed.

CERTIFIED NURSING ASSISTANTS: A skillful hospice nursing assistant will provide assistance with the personal hygiene for our patients. They will assist patients and their families with routine bathing, grooming and provide assistance in making their environment more comfortable. Our certified nursing assistants offer everything from a warm shave to aroma therapy oils. The frequency of visits can vary based on need, but most patients benefit from two to three visits a week.

SUPPORT SERVICES: The unique benefit of hospice care is being surrounded by people that can support you during a unique time in your life. Our social workers, chaplains and bereavement specialists are here for you and your family throughout your journey. Our social workers are experts in knowing community resources, providing counseling and simply lending an attentive ear. Our chaplains are trained in all areas of spirituality and can be involved as much or as little as you find helpful.

VOLUNTEERS: Hospice de la Luz has an amazing group of committed, trained and experienced volunteers to provide companionship and in home support to our hospice patients. Our volunteer program goes above and beyond what you can expect from other hospice organizations.

PHYSICIAN SERVICES: You are able to maintain seeing your current physician as you add hospice services to your healthcare plan. Our hospice will provide you a trained hospice physician that can visit you at home or anywhere you may be residing.
